• 1 photograph measuring 37×52 mm,
• 1 electronic photograph with dimensions of 200×250 mm, 600 dpi,
• in the case of foreigners – a document confirming knowledge of the Polish language or proof of payment for a Polish language course in the case of applying for admission to studies conducted in Polish,
